In our practice, we use liquid, biodegradable hyaluronic acid fillers with varying degrees of cross-linking, biosynthetically produced through microbial fermentation. The degree of cross-linking in hyaluronic acids affects the viscosity and lifting capacity of the product. Lightly cross-linked hyaluronic acids are more fluid, with lower lifting capacity, primarily enhancing water retention. Highly cross-linked hyaluronic acids are gel-like, with high lifting capacity, making them ideal for correcting volume deficits. Hyaluronic fillers can be injected deep into the tissue using sharp cannulas in what is known as the depot technique, ideal for volume enhancement or facial contouring. For more superficial wrinkles or tear trough filling, fanning techniques with blunt cannulas are excellently suited. This approach minimizes the risk of injuring blood vessels.
To prevent pain, we apply a topical anesthetic gel beforehand. After a thorough analysis of the facial contours, we inject the appropriate hyaluronic acid directly under the skin in areas of volume loss. The products we use also contain local anesthetics, making the injections barely noticeable.
Small bruises and swelling may occur at the injection sites. However, these typically subside within a few days and can be immediately covered with makeup after treatment. For hyaluronic acid injections in the lip area, visible swelling may appear in the first 48 hours but will gradually diminish.
Effects are visible immediately after treatment. Due to the water-binding capacity of the hyaluronic preparation, these effects intensify within the first few weeks. The treatment lasts approximately four to six months, after which the body naturally breaks down the hyaluronic acid. The filler effect in the lips and tear trough area can last up to twelve months.
Hylase, or hyaluronidases, are enzymes that break down hyaluronic acids. They are available in various concentrations and are dissolved in saline solution. To dissolve misplaced hyaluronic acid deposits or reduce swelling after injections, we can inject small amounts of the enzyme with very fine needles in the targeted areas. The result is immediately visible and continues to improve slightly over the following hours.
We use Hylase when hyaluronic acid injections have caused significant swelling or deformities. Many practitioners are unfamiliar with the proper ratios and may misuse hyaluronic acid. This can happen, for example, if a highly water-retentive, viscous hyaluronic acid product is used. If you’re dissatisfied with the treatment outcome, hyaluronic acid can be dissolved using Hylase. The concern that Hylase "breaks down too much" hyaluronic acid is unfounded. Studies have shown that Hylase actually stimulates fibroblasts, encouraging them to produce hyaluronic acid independently.
