The purpose of the saphenous veins in the leg (V. saphena magna, V. saphena parva) is to transport blood against gravity back to the heart. The venous valves, located along the veins, support this return flow. Due to genetic predisposition, pregnancy, or increased static strain, the venous valves weaken over time, leading to reduced blood flow. As a result, pressure in the vein increases, causing blood pooling, which leads to dilation and bulging in the superficial veins. These visible bulges are known as varicose veins (varicosis). This condition can also lead to pain, skin changes, and evening swelling of the lower legs.

Spider veins are a subtype of varicose veins (varicosis) and appear in a web-like pattern in the superficial layer of the skin. Due to blood pooling in the venous system, familial predisposition, and other risk factors (standing or sitting for long periods, pregnancy, obesity, hormone treatments), the connective tissue can weaken. Since the venous system primarily consists of connective tissue, this causes the vessels to dilate. When small superficial skin vessels are affected, we refer to them as spider veins. Before treating spider veins, it’s important to rule out any underlying deeper venous system conditions (CVI, chronic venous insufficiency). Spider veins can be effectively treated. During the painless examination of the saphenous veins, we use modern ultrasound techniques (Doppler and color-coded duplex ultrasound). This allows us to directly identify enlarged veins and assess any damage to the venous valves. After the examination, we can discuss and develop an individualized treatment plan with you. For the treatment and removal of spider veins, depending on the diagnosis, options include sclerotherapy (foam sclerotherapy) or laser treatment. Usually, multiple sessions (2–4 treatments every 2–4 weeks) are required to achieve a satisfactory aesthetic result. Before any treatment, a thorough ultrasound examination of the venous system is always performed to rule out underlying venous disease.
