How much hair you are losing is difficult to assess simply by looking in the mirror. It can, however, be measured. With AI-assisted hair analysis, we record hair density, hair thickness and the growth phases of your hair as objective values – making changes visible long before you notice them yourself.

What Is an AI-Assisted Hair Analysis?

With AI-assisted hair analysis – often also referred to as digital trichoscopy – a high-resolution camera captures a highly magnified, standardised image of your scalp. Trained image recognition software then evaluates this image automatically: it identifies each individual hair, distinguishes it from the skin surface, measures its diameter and assigns it to the corresponding follicular opening. What once involved counting hairs under a microscope – prone to error and varying from one examiner to another – has now become a reproducible measurement. At our practice in Hamburg, we use the FotoFinder ATBM master and TrichoScale AI analysis software for this purpose.

One point is particularly important to us: the artificial intelligence measures, but it does not diagnose. It provides precise data. Gentle Procedure – Usually Without Shaving

The traditional examination of the hair roots, known as a trichogram, required around fifty hairs to be pulled out. In most cases, this is no longer necessary today. For hair density, hair thickness, the vellus-to-terminal ratio and follicular units, a single image is sufficient – without shaving, plucking or preparation. You can attend your appointment with normally washed and styled hair. Only if we also want to determine the anagen-to-telogen ratio do we trim an area of approximately one square centimetre in a concealed location and photograph it again after two to three days. The area is positioned so that the surrounding hair completely covers it.

Medical History

We discuss the progression of your hair loss, your medical history, hormonal situation, medications and lifestyle. We then assess the scalp using a digital dermatoscope.

Standardised Imaging

The measurement area is marked so that every subsequent measurement captures exactly the same location. The imaging itself takes only a few minutes and is painless.

AI Analysis

The software analyses the image and calculates all parameters within seconds. This also makes it easier to compare images at later follow-up appointments.

An Additional Tool for Analysis: Mobile Trichoscopy with skeen

In addition to the ATBM master, we work with skeen, FotoFinder’s wireless digital dermatoscope with 20x and 40x magnification, both polarised and non-polarised. This allows us to assess the scalp and follicles directly at the treatment chair: signs of inflammation, perifollicular redness, scaling and scarring – including at the temples, nape, crown or eyebrows. The images are evaluated with AI support and stored in encrypted form. The two systems complement each other: the ATBM master provides the standardised, quantitative measurement required for follow-up monitoring, while skeen allows rapid qualitative assessment at any area of the scalp.

Why Follow-Up Monitoring Makes the Difference

Hair treatments work slowly. There are usually three to six months between the start of treatment and a visible result – a period during which many patients become uncertain and discontinue treatments that are actually beginning to work. This is precisely where measurement becomes valuable. Because we always assess the same marked area, the values can be compared directly. If hair thickness increases, the proportion of telogen hairs decreases or density improves, these changes become apparent in the measurements much earlier than in the mirror. Conversely, we can also identify early on when a treatment concept is not working and adjust it instead of losing several months. We recommend follow-up measurements after 3, 6 and 12 months.

When Measurement Is Not Enough: Scalp Biopsy

Not every form of hair loss can be reliably classified using trichoscopy alone. If there is suspicion of scarring alopecia – such as lichen planopilaris, frontal fibrosing alopecia or folliculitis decalvans – we confirm the diagnosis with a scalp biopsy and histological examination. The procedure takes only a few minutes and is performed under local anaesthesia in our procedure room. In scarring forms of hair loss, this step can determine whether the inflammation can be stopped before follicles are permanently lost. That is why we do not delay it when the findings indicate that it is necessary.

Frequently Asked Questions About AI Hair Analysis

What Is the Difference Between Trichoscopy, TrichoScan and a Trichogram?

Trichoscopy is the examination of the hair and scalp under high magnification. TrichoScan is the computer-assisted, now AI-based analysis of such images, including the calculation of objective measurements. A traditional trichogram, by contrast, required hairs to be plucked out so that the roots could be examined under a microscope – this method has now largely been replaced by digital analysis.

Do I Need to Have My Hair Shaved for the Hair Analysis?

In most cases, no. We can assess hair density, hair thickness and the vellus-to-terminal ratio from a single image without shaving. Only for the anagen-to-telogen ratio do we trim an approximately 1 cm² concealed area and photograph it again after two to three days.

Is the Examination Painful?

No. The imaging is carried out with minimal contact on the scalp, without anaesthesia and without any after-effects.

How Do I Need to Prepare?

No special preparation is required. You can come with normally washed and styled hair. There is no need to avoid hair cosmetics. If you have recent laboratory results, you are welcome to bring them with you.
