Wrinkle Smoothing Through the Body’s Own Collagen – JULÄINE™ Hamburg
Sometimes the face does not lack anything that needs to be filled. The contours are still well defined, yet the skin appears thinner than it used to and more tired in the evening, as though it has lost its firmness over the course of the day. In such cases, the issue is usually not a lack of volume, but a lack of collagen, which the body produces more slowly as the years go by.
With JULÄINE™, we therefore do not aim for an immediate volumising effect: the injectable collagen biostimulator contains poly-L-lactic acid, or PLLA, and supports the body in gradually building new collagen. As we offer several biostimulators at our private dermatology practice in Hamburg, we will first assess honestly whether JULÄINE™ is suitable for your skin or whether another treatment would be the better choice.
The Treatment Process
Consultation
First, we discuss your wishes, examine the tissue and record any previous injectable treatments. We then assess whether JULÄINE™ is suitable for your individual findings and which treatment plan appears medically appropriate.
Preparation
The skin is carefully cleansed and disinfected, and the intended injection areas are marked. Whether an anaesthetic cream is additionally used depends on your preferences and your individual sensitivity to pain.
Injection
The product is injected in a fan-shaped pattern into the deep dermis or superficial subcutaneous tissue using a fine needle, allowing it to distribute evenly. Additional massage is not necessary in this case.
Aftercare
Cooling can help reduce swelling during the first few days. It is best to avoid saunas, solariums and intensive exercise until any possible after-effects have subsided. The next session takes place after approximately 6–8 weeks and is prepared and monitored accordingly by us.
What is JULÄINE™?
JULÄINE™ of Sweden belongs to the group of injectable collagen biostimulators and is classified as a Class III medical device. The product contains microparticulate poly-L-lactic acid, or PLLA – a biodegradable material that has been used in various areas of medicine for many years.
Unlike a conventional hyaluronic acid filler, it is not primarily intended to replace lost volume immediately with a gel. Instead, the PLLA particles support processes within the treated tissue that can lead to the formation of new collagen over several weeks. The treatment result therefore develops gradually.
Its intended purpose includes the correction of shallow to deep nasolabial folds in adult, immunocompetent individuals. Whether JULÄINE™ may also be suitable in your case for off-label use, meaning outside its intended purpose, is something we assess during our consultation.
How JULÄINE™ Works
JULÄINE™ contains tiny microspheres made of poly-L-lactic acid. This substance has been known in medicine for decades, for example from absorbable sutures. The microspheres are introduced into the deeper layers of the skin.
Your body recognises the microspheres as “foreign”. This is not a side effect, but precisely the mechanism of action. The body’s own immune cells, known as phagocytes, gather around the particles. Because the microspheres are too large to be removed easily, these cells switch into a kind of “repair mode”. They release signalling substances that activate the collagen-producing cells in the skin. These then produce new, naturally occurring collagen over the course of weeks and months.
This reaction is very mild and is normally neither noticeable nor visible. That is why the result does not appear immediately, but develops gradually.
The microspheres themselves are slowly broken down by the body over many months, ultimately into water and carbon dioxide. What remains is not the material itself, but your own newly formed tissue.
Using a special technology called LASYNPRO™, the manufacturer has designed the microspheres to be particularly uniform and smooth. This is intended to help ensure that the body’s response occurs evenly and can be well controlled.
Who is JULÄINE™ Suitable For?
When It Is Not Suitable
A biostimulator reaches its limits in cases of pronounced skin laxity. Tissue that has lost significant support cannot be fully restored through new collagen alone – in such cases, surgical procedures or other active substances may, for example, be more suitable. We discuss this openly during the initial appointment.
JULÄINE™ is also not suitable if you are looking for visible changes by a specific date, as the effect develops gradually. In that case, treatment with a hyaluronic acid filler may be more appropriate.

Side Effects and Contraindications
The Days Following Treatment
- Redness and swelling in the treated area, usually lasting one to three days
- Tenderness when placing a hand on the area or when sleeping on your side
- Occasionally, a small bruise at the injection site
- In a small proportion of patients, swelling lasts for more than five days
Possible Nodules
- With biostimulators, nodules can develop when particles are distributed unevenly within the tissue
- In a Spanish observational study involving 95 patients, neither nodules nor circulatory disturbances occurred within six months
- These findings should be interpreted with caution: the group was small, there was no control group, and no data are available beyond six months
- Rare reactions cannot be ruled out with any injectable procedure
Contraindications
- Active inflammation or infection in the intended treatment area
- Known hypersensitivity to any of the product’s components
- Pregnancy and breastfeeding
- Particular caution in patients prone to keloid scarring and in those with autoimmune or connective tissue disorders
- Areas in which a permanent filler material has already been injected
- We discuss blood-thinning medication in advance, as it can increase the likelihood of bruising

If you find fine lines bothersome but do not want an immediate filling effect, gradual collagen stimulation may generally be an option. During a consultation at our private practice in Hamburg, we assess the density and firmness of your skin, listen to what matters to you, and explain openly what a biostimulator such as JULÄINE™ can and cannot achieve.
Sometimes the outcome of this consultation is that hyaluronic acid will achieve the desired result more quickly or that Sculptra® is the more suitable choice. Because we offer all of the treatments mentioned here, we can give you this recommendation without any ulterior motive.
